  2.   HMO members over 65 years of age should remember that the HMO plan replaces Medicare coverage and
  3.   you must stay within your HMO plan for your claims to be paid by Medicare.
  4. ‚Ä¢ PPO (Preferred Provider Organization), EPO (Exclusive Provider Organization) or IPA (Independent
  5.   Practice Association). Traditionally, these are groups of physicians and other participating providers who
  6.   have agreed to offer a discount from their usual fees in order to participate in a particular group. These
  7.   function as "old-fashioned private practice" in that the physician or other provider receives a fee each time a
  8.   service is performed or each time you see the physician. You do have a choice of using providers or physicians
  9.   who are not in the plan, but if you do, you will have to pay a larger portion of the cost.
